No need to make a phone call - you can now make your appointment online! Clicking the button below will take you to our secure online appointment system.

New Patients: You must FIRST register to use this system by clicking on the link below. Once you register, we will review your registration within a few hours, and then send you an email activating your ability to schedule online. Once you receive your activation email, return to this page and make your first appointment.

Please understand that appointments after 3:00 pm may take 4-8 weeks to schedule. The pathway to scheduling an after school or evening time is to register through the online appointment link below and place yourself on the waitlist.  On the waitlist, let us know the day(s) and time(s) you are available.  We fill available late afternoon and evening appointments for new patients from the waitlist. 

If something changes in your schedule, remember that your appointment confirmation email allows you to cancel or reschedule your appointment online too!

Use of online appointment scheduling indicates that you understand and agree to the following terms:

1. The use of online appointment scheduling may not be confidential or secure although the site is secure, encrypted and monitored for security.  The onlne appointment system does not contain any health protected information.
2. Online appointments cannot be made for the same day.
3. Only one appointment can be scheduled at a time. 
4. We will notify you as soon as feasible if we cannot honor your appointment due to unforeseen scheduling conflicts or commitments.
